Your provider’s Family Health Team is made up of a number of family physicians, nurse practitioners and inter-professionals who are working together to give you and your family better access to quality primary healthcare services.
Your provider reserves appointments during the day and in the evening for your urgent healthcare needs.
Providers within the Windsor Family Health Team, who are a part of your circle of care, will be able to access your medical record, should they need to, to provide healthcare services.
